What is Medial Collateral Ligament (MCL) Injury?

The Medial Collateral Ligament (MCL) is a strong band of tissue located along the inner side of the knee. It connects the femur (thigh bone) to the tibia (shin bone) and helps provide stability to the knee joint.

One of its main functions is to resist valgus stress, which is a force that pushes the knee inward.

An MCL injury occurs when the ligament is stretched beyond its normal capacity or partially or completely torn.

MCL injuries are commonly graded according to severity:

Grade 1 MCL Injury: The ligament is stretched but remains intact. Symptoms are usually mild.

Grade 2 MCL Injury: The ligament is partially torn, causing greater pain and some knee instability.

Grade 3 MCL Injury: The ligament is completely torn, which can result in significant instability and may occur alongside other knee injuries.

MCL injuries may occur alone or together with injuries involving other structures such as the ACL, meniscus, or other knee ligaments.

Common Symptoms

Inner Knee Pain: Pain is usually felt along the inside of the knee.

Tenderness: The area over the MCL may be painful when touched.

Swelling: Swelling may develop around the knee after the injury.

Knee Stiffness: Pain and swelling may make bending and straightening difficult.

Pain with Movement: Walking, twisting, or changing direction may increase discomfort.

Feeling of Instability: The knee may feel less stable, particularly with a more significant injury.

Other Possible Symptoms

Bruising: Discolouration may develop around the inner side of the knee.

Difficulty Bearing Weight: Standing or walking may be painful after the injury.

Pain with Twisting: Rotating movements can aggravate symptoms.

Pain During Direction Changes: Cutting or pivoting movements may feel painful or unstable.

Reduced Range of Motion: Knee movement may become limited due to pain and swelling.

Muscle Weakness: The thigh muscles may weaken during recovery.

Difficulty Walking: You may limp or avoid fully loading the affected leg.

Physical Examination: The knee is assessed for pain, swelling, tenderness, movement, and stability.

Valgus Stress Testing: A controlled examination may assess the stability of the MCL.

Range of Motion Assessment: Knee bending and straightening are evaluated.

Strength Testing: The quadriceps, hamstrings, hips, and other supporting muscles may be assessed.

Functional Assessment: Walking, balance, stepping, squatting, and other movements may be assessed when appropriate.

X-Rays: X-rays may be used to rule out associated bone injuries or fractures.

MRI Scan: MRI can help assess the severity of the MCL injury and identify associated injuries to other ligaments, cartilage, or the meniscus.

Ultrasound: Ultrasound may be used in selected cases to assess superficial soft-tissue structures.
